Understanding the complex dynamics of addiction and mental illness, the Follow Up for Success program focuses on individuals and families who have received treatment for substance abuse or mental illness. Six months of intensive services, including home based case management, life skills training, employment and financial counseling, in addition to short term financial assistance for housing-related costs, are the keys to success for this program. All of these specialized services provide a stable foundation from which families and individuals grow into self sufficiency.  75 households were served in 2006 and of all who completed the program, 100% exited to permanent housing.
