The largest of its kind in Kentucky, the HIV/AIDS Care Coordinator Program serves over 700 clients each year.  The Care Coordinator Program offers case management and referral services to people living with HIV/AIDS in Jefferson County and the six surrounding counties. The purpose is to help the client maintain the highest quality of life while managing the disease. The services provided for the client include but are not limited to intake and assessment, goal setting, conflict resolution, crisis intervention, referral to community services, emergency financial assistance, linkage to rental and utility assistance, entry into support groups, mental health and linkage to community resources.
