The Incarcerated Veterans Transitional Program facilitates the successful reintegration of veterans exiting the corrections system into their home communities.  Engaging individuals before release from a state penal institution or timed with release, this program incorporates a customized case plan for each participant.  Clients may access transitional housing, employment services, linkage with mental health and substance abuse services, as well as ongoing follow up.
